NO WEEKENDS OR HOLIDAYS

Part-Time (less than 30 hours/week) or Full-Time (30-37.5 hours/week)
Starting Wage: $21.00/year, based on individual's education and experience

This position requires work to be completed in client homes and in an office setting (Dane County, WI area), with the possibility of completing some work remotely once job-competency has been demonstrated.

Primary Duties:

  • Work with clients to develop and teach strategies to promote coping skills, recovery principles, wellness, and independence
  • Provide support for individuals working toward recovery using a supportive, strengths-based, and culturally humble approach to treatment
  • Facilitate community integration, participation, and housing access; assisting clients with building connections and resources
  • Utilize effective strategies to engage program participants, enhance motivation, and encourage meaningful goal-planning and progress
  • Conduct resource referrals and assist with coordinating life transitions
  • Provide screening and assessments, service linkage, and service facilitation on the CCS service array
  • Provide mediation, lifestyle coaching, and advocacy with individuals and providers
  • Conduct in-home assessments and provide follow-up support to clients

Minimum Education and Experience:

  • Bachelor's Degree in Mental Health, Human Services, or related field, required; Master's Degree desired
  • Comprehensive Community Services (CCS) credential in Dane County WI, strongly desired
  • 2 years experience in Case Management and working with individuals with intellectual disabilities, physical disabilities, chronic health conditions, or seniors, strongly desired
